Request for Help
IMPORTANT – Read this First: To help us make sure that the clients with the greatest need are receiving the help they need, please check our Consumer Resources page before submitting a request for assistance. It’s possible the solution to your situation is something simple you can take care of today once you’re armed with the right information!
If you’ve already checked the Consumer Resources page and still need assistance from a Consumer Assistance Program team member, please fill out the short form below and one of our staff or volunteers will get back to you as soon as possible. Please note that we do not have financial resources to help pay medical bills.
While we strive to respond to new inquiries within five (5) business days, due to a tremendous increase in inquiries and staffing limitations, it may take longer. Please be patient.
What happens after you submit a request for help?

A CAP team member will contact you by phone and/or email within five (5)* business days to get more information.
- We’ll then send a follow up email with a summary of what we discussed, what you’d like us to help you with, and an authorization form.
- We may request documents such as bills, explanations of benefits, and insurance information and we will provide ways to securely share them with us.

From there, we’ll work with you to develop a plan to come to the best possible resolution, and our staff or volunteers will follow up with you regularly throughout the duration of your case.

Once your case is closed, you’ll receive a follow up survey to see how we did, be provided with opportunities to get more involved in CCHI’s legislative and policy work, and we’ll make a small donation request.
- This program is free and you do not need to donate to receive services. Our program relies on donations from our community to keep this free service available, though, so your generosity is very much appreciated!
*Sometimes we have a higher than usual number of cases, so it may take us longer than three (3) business days to respond to your request. This page is updated regularly with the most recent wait times. We only have three staff and three volunteers, so we appreciate your patience as we work our hardest to serve the community!
